Job summary

Ontario Center for Rehabilitation and Healthcare in Canandaigua, NY, part of Centers Health Care, seeks an experienced MDS Coordinator to manage MDS assessments and care plans.

The position requires MDS 3.0 experience, a New York RN license, and a long-term care background, plus strong organization, communication, and teamwork.

You will work with interdisciplinary teams to ensure compliant documentation and timely care.

Qualifications

Responsibilities

• Completing accurate assessments, MDS & care plans as assigned

• Monitoring & auditing clinical records, ensuring accuracy & timeliness
• Informing DON of persistent issues related to non-compliant documentation
