Job summary

CareCore Health in Cincinnati, OH is seeking a full-time MDS Coordinator to join our team on-site. You will ensure compliance with federal and state MDS guidelines, manage Medicaid/Medicare RUGs/PDPM, and prepare documentation for audits and quality measures.

The ideal candidate is a licensed nurse in Ohio (RN preferred) with MDS 3.0 knowledge, familiarity with EHR/MDS software, and strong communication, organization, and teamwork skills. Prior year experience is preferred.

Qualifications

• Licensed RN in Ohio preferred and eligible to practice
• RAC‑CT certification preferred to support MDS processes
• Minimum of 1 year of experience as an MDS Coordinator or equivalent
• Strong understanding of MDS 3.0 guidelines and SNF regulations
• Knowledge of Medicare/Medicaid reimbursement processes
• Excellent communication, organizational and time management skills
• Proficiency with EHR and MDS software systems
• Ability to work collaboratively in a team environment

Position Overview:

The MDS Coordinator will play a crucial role in ensuring compliance with federal and state guidelines related to the Minimum Data Set (MDS), and will be responsible for managing Medicaid and Medicare RUGs/PDPM, Medicaid Case Mix, and meeting Medicare criteria, including all required supportive documentation. A strong understanding of Quality Measures and the Five-Star Quality Rating System is essential.

Qualifications:

• Licensed Nurse in the State of Ohio (RN Preferred)
• RAC-CT Certification Preferred
• Minimum of 1 year of experience as an MDS Coordinator or equivalent role
• Strong understanding of MDS 3.0 guidelines and skilled nursing facility regulations
• Knowledge of the Medicare and Medicaid reimbursement process
• Excellent communication, organizational, and time management skills
• Proficiency in electronic health records (EHR) and MDS software systems
• Ability to work collaboratively in a team-oriented environment
