Little Village Nursing in Chicago, IL seeks an MDS Coordinator to lead resident MDS assessments in compliance with state and federal regulations. You will collaborate with the Care Plan Coordinator and IDT to ensure timely and accurate scoring and documentation.

The ideal candidate is a licensed RN or LPN in Illinois, fluent in English, with strong leadership and knowledge of long-term care regulations. This full-time role requires coordinating care plans, triggers, and monthly documentation

Formación

• Active Illinois RN or LPN license.
• English language proficiency (read, write, speak).
• Ability to coordinate interdisciplinary care conferences and Medicare meetings.
• Ability to perform thorough assessments of all residents.
• Ability to complete documentation for MDS per regulations.
• Confidentiality must be maintained.
• Ability to interact tactfully with staff, residents, families and agencies.
• Knowledge of nursing and medical practices in long-term care.
• Leadership and supervisory ability required.
• Must be physically capable of hearing, seeing and speaking.

Responsabilidades

• Coordinate with Care Plan Coordinator to complete MDS in compliance with State and Federal requirements.
• Calculate triggers and develop resident assessment protocols for care plans.
• Ensure MDS are completed timely and accurately reflect resident functioning.
• Identify lowest functioning level allowable to maximize reimbursement within laws.
• Ensure CNAs, nurses and IDT complete required clinical documentation for MDS scoring in a timely manner.
