Bartlesville Health & Rehab Community is hiring an MDS Coordinator to join our leadership and nursing team. This role emphasizes accurate assessments, documentation, care planning, and reimbursement processes.

You will be pivotal in aligning resident care with regulatory requirements. The MDS Coordinator coordinates assessments, manages schedules, reviews documentation, and collaborates with nursing, therapy, dietary, social services, admissions, and the business office to ensure efficient care

Qualifications

• Current Oklahoma RN license is required or expected.
• Strong communication skills and ability to document clearly.
• Good computer skills and experience with skilled nursing/long-term care.
• MDS experience strongly preferred.

Responsibilities

• Coordinate resident assessment process in skilled nursing/long-term care.

MDS Coordinator

Bartlesville Health & Rehab Community is hiring an MDS Coordinator to join our leadership and nursing team. We are looking for a dependable, detail-oriented, and organized nurse who understands the importance of accurate assessments, documentation, care planning, reimbursement, and interdisciplinary communication.

The MDS Coordinator helps coordinate the resident assessment process in a skilled nursing and long-term care setting. This includes managing MDS schedules, completing and coordinating assessments, reviewing documentation, supporting care plan accuracy, monitoring assessment deadlines, communicating with nursing, therapy, dietary, social services, activities, admissions, and the business office, and helping ensure documentation supports resident care and reimbursement requirements.

This position plays an important role in resident-centered care, quality measures, survey readiness, Medicare and Medicaid reimbursement, skilled documentation, and communication between departments. A successful MDS Coordinator is accurate, organized, self-motivated, calm under deadlines, comfortable reviewing clinical documentation, and able to work well with the interdisciplinary team.

Applicants should have a current Oklahoma nursing license, strong communication skills, good computer skills, and experience in skilled nursing or long-term care. MDS experience is strongly preferred.
