Bartlesville Health & Rehab Community is hiring an MDS Coordinator to join our leadership and nursing team. This role emphasizes accurate assessments, documentation, care planning, and reimbursement processes.

You will be pivotal in aligning resident care with regulatory requirements. The MDS Coordinator coordinates assessments, manages schedules, reviews documentation, and collaborates with nursing, therapy, dietary, social services, admissions, and the business office to ensure efficient care

Qualifications

• Current Oklahoma RN license is required or expected.
• Strong communication skills and ability to document clearly.
• Good computer skills and experience with skilled nursing/long-term care.
• MDS experience strongly preferred.

Responsibilities

• Coordinate resident assessment process in skilled nursing/long-term care.
• Manage MDS schedules and complete assessments on time.
• Review documentation and support care plan accuracy.
• Monitor assessment deadlines and compliance with regulations.
• Communicate with nursing, therapy, dietary, social services, admissions, and business office.
• Ensure documentation supports resident care and reimbursement requirements.
