Job summary

Westminster Nursing and Rehab is seeking a dedicated MDS Coordinator to join our team in Westminster, MD. This part-time role focuses on coordinating the Minimum Data Set process to ensure residents receive accurate assessments and care planning.

You will collaborate with an interdisciplinary team, review records, and contribute to regulatory compliance and quality improvement initiatives to enhance resident outcomes.

Qualifications

• High school diploma or equivalent required; degree preferred.
• Experience in long-term care or rehabilitation preferred.
• Strong organizational, communication, and analytical skills.
• Familiarity with MDS process and software is a plus.

Responsibilities

• Coordinate the MDS process, including reviewing and updating resident records, conducting assessments, and ensuring accuracy and completeness
• Work closely with interdisciplinary team members to gather information and complete MDS assessments
• Analyze and summarize MDS data to identify trends and areas for improvement
• Develop and implement plan of care based on MDS assessment and resident goals
• Collaborate with healthcare team members to ensure completion of care plans and coordination of services
• Maintain accurate and up-to-date documentation and ensure compliance with regulatory requirements

We are seeking a dedicated and detail-oriented MDS Coordinator to join our team at our Westminster, MD location. As a Part-Time MDS Coordinator, you will play a vital role in ensuring the well-being and quality of care for our residents by coordinating the Minimum Data Set (MDS) process.

Responsibilities
