Job summary

SNF Staffing Solutions is hiring a MDS/Clinical Coordinator in Benson, AZ. You will ensure timely completion of the RAI/MDS process from admission to discharge, coordinating with the Director of Nursing and broader clinical leadership.

The role requires LVN licensure, familiarity with Medicare/Medicaid reimbursement, and strong documentation skills to maintain compliance and quality of care.

Qualifications

• Interdisciplinary collaboration with care teams.
• Knowledge of federal/state regulations and Medicare/Medicaid rules.
• Experience with MDS 3.0 preferred.
• Accurate data entry and documentation in MDS systems.

Responsibilities

• Coordinate MDS assessments and care plan reviews with the Interdisciplinary Team.
• Decide on additional MDS assessments based on clinical criteria and MDS guidance.
• Organize daily stand-up meetings to review resident care and assessment dates.
• Ensure compliance with federal/state regulations in MDS processes.
• Monitor MDS and care plan documentation in medical records.
• Maintain current MDS status per guidelines and accurate data entry.
• Code MDS accurately from medical records and interviews.
• Attend team meetings to communicate changes and update records.
• Assist with significant changes and physician orders reflected in MDS and care plans.
• Prepare scheduling and notifications for care conferences with DON.
• Keep knowledge base updated on data entry and tech tools.
• Submit required documentation to state databases and other entities.
• Follow up on MDS submissions and validation errors.
