Job summary

Majestic Care of Whitehall is seeking an MDS Assistant (RN/LPN) to supervise and coordinate MDS and care plans for residents in our facility, ensuring timely and accurate documentation.

The role requires an active RN or LPN license in the state, knowledge of MDS 3.0, and prior long-term care experience. Strong data-entry and planning skills are essential for successful PPS and state filings.

Qualifications

  • Must hold current RN or LPN nursing license in the state of employment; license must be active, valid, and in good standing.
  • Experience in Long Term Care preferred.
  • Computer skills such as Data Entry and Word processing required.
  • Working knowledge of the MDS 3.0 (current version of minimum data set).
  • Must be knowledgeable of nursing and medical practices, as well as laws and guidelines pertaining to skilled nursing facilities.
  • Must possess the ability to plan, organize, develop, implement and interpret programs and policies that provide quality care.

Responsibilities

  • Coordinate MDS and Care Plan as outlined by facility policies and procedures.
  • Inform care team when a care plan is due and ensure completion.
  • Develop or revise resident care plans quarterly or with condition changes.
  • Assist with PPS and state MDS assessments as required.
  • Ensure residents have MDS, CATs, and CAAS completed.
  • Participate in care plan meetings as directed.
  • Assist with MDS submissions to state/federal agencies per guidelines.
  • Maintain MDS Schedule dates and ensure timely completion.
  • Observe resident rights, confidentiality, and privacy in MDS processes.
  • Maintain cross-department communication for updated MDS/care plans.
  • Monitor care during MDS and report concerns to leadership.
  • Assist with pre-admission screening as directed.
  • Report discrepancies between observed status and medical records to DON.

Position Overview:

The MDS Assistant participates in supervising and coordination of all MDS (Minimum Data Sets) and care plans for residents within the facility in a timely and factual manner and in accordance with the state requirements.


Key Responsibilities:


  • At the direction of the MDS Coordinator conducts and coordinates the MDS and Care Plan as outlined by the facilities’ policies and procedures.

  • Assists with informing all care team members of when a care plan is due and ensuring that all care team members complete their portion of the MDS.

  • Develop and/or revise resident care plan quarterly and with any significant change in condition.

  • Assist with completion of all PPS and state Minimum Data Set required assessments.

  • Ensure that all residents have the appropriate MDS, CATs, and CAAS completed.

  • Participates as directed by MDSC or ED in care plan meetings.

  • As directed by the MDSC submission of MDS to state/federal agencies according to Medicare/Medicaid guidelines.

  • Assist as directed by the MDSC with the maintenance of the Resident Assessment and Care Plan Schedule ensuring all dates for MDS and CAAS completion are met in accordance with state/federal guidelines.

  • Ensures resident’s rights are observed in the MDS Process, such as confidentiality and privacy.

  • Maintain effective communication among departments so MDS and care plans can be kept current when changes to the residents’ condition occur.

  • Monitor resident care during the MDS process and reports concerns to the Facility Executive Director and Director of Nursing Services.

  • Assists with the pre-admission screening process as directed by the MDSC or ED.

  • Reports contraindications between observed resident status and documentation in the residents’ medical record to the Director of Nursing Services.
