Job summary

Derry Rehabilitation and Nursing Center is seeking an RN with MDS 3.0 experience to join our team. You will complete assessments, care plans, and maintain documentation for residents in a collaborative setting.

The role requires NYS RN license, 2 years MDS 3.0 and long-term care experience, strong organizational and communication skills, and comfort with computer systems.

• MDS 3.0 experience (2 years) required
• Long-Term Care experience (2 years) required
• Highly organized, professional and motivated

Responsibilities

• Completing accurate assessments, MDS & care plans as assigned
• Initiating care plans and supporting activities as assigned
• Creating and distributing monthly care plan calendars in a timely fashion
• Maintaining & updating all care plans and assessments as required
• Monitoring & auditing clinical records, ensuring accuracy & timeliness
• Informing DON of persistent issues related to non-compliant documentation
• Protecting the confidentiality of Resident & Facility information at all times

Requirements:

• MUST HAVE PRIOR MDS 3.0 EXPERIENCE
