Job summary

Merwick Post Acute is seeking an experienced MDS Coordinator to join our in-office team. An active RN license in New Jersey is required, along with a minimum of three years’ long-term care experience and prior MDS/RAI familiarity.

You will ensure accurate MDS assessments, maintain compliance, and support resident care planning with interdisciplinary teams. Join a 200-bed facility dedicated to compassionate, compliant care, with opportunities for growth and a supportive work environment.

Qualifications

• Graduate of an accredited School of Nursing (RN, BSN, or LPN).
• Current, active RN license required in NJ.
• Minimum of three years of clinical experience in a long-term care setting.
• Prior MDS/RAI experience required.
• Strong clinical assessment skills with attention to detail and accuracy.
• Working knowledge of Medicare and Medicaid regulations and reimbursement systems.

Responsibilities

• Ensure timely and accurate completion of all MDS assessments per regulatory requirements and deadlines.
• Verify compliance with federal, state, and facility regulations related to MDS documentation and submission.
• Supervise MDS data entry, validation, and transmission processes.
• Identify, investigate, and resolve data discrepancies and validation issues.
• Prepare and present MDS-related reports to the DON and leadership.
• Provide feedback to clinical teams on documentation and care planning.
