Job summary

Mosaic Life Care is seeking a Clinical Coordinator to support MCCN activities, coordinating e-Consults and referrals with Mayo Clinic, educating providers on MCCN tools, and ensuring seamless clinical collaboration. The role emphasizes cross-functional coordination, meeting participation, and ongoing education of care teams.

The position requires an RN license, a Bachelor’s in Nursing (preferred), and at least 3 years of clinical experience.

Qualifications

• Must hold RN license and be in good standing.
• Ability to read, write, and communicate clearly.
• Strong computer and telecommunication skills for care coordination.

Responsibilities

• Assist in requesting e-Consults and coordinating MCCN referrals.
• Educate providers about MCCN tools and processes.
• Submit eConsults and referrals via Carelink; coordinate clinical activities.
• Collaborate with MCCN Business Coordinator on meetings and projects.
• Attend monthly and quarterly MCCN governance meetings; support communications.
