Job summary

Qualifications

• Graduate of a Nursing program; higher degree may be required for some hires.
• 3 years of acute nursing preferred; 1 year of Case Management preferred.
• Certification (CCM or ACM-RN) preferred.
• Current RN license by applicable state; Basic Life Support required.
• Additional state licensure and/or specialty certification may be required.

Responsibilities

• Coordinate patient plan of care across the continuum and manage utilization.
• Lead within the Mayo Clinic Nursing Practice Model; educate on case management concepts.
• Bridge communication between providers, patients, families, and outside agencies.
