Job summary

Malama Health seeks a Community Doula in Colorado to serve as a Maternity Care Navigator, delivering care management and outreach to pregnant and postpartum members. You will partner with clinics, CBOs, and providers to ensure timely access and adherence to care protocols while building trust in local communities.

Ideal candidates are bilingual in English and Spanish, with 1–2 years in community health work, and comfortable traveling throughout assigned counties to engage families at WIC sites,

Qualifications

• 1–2 years of experience in Community Health Worker/related role.
• Must be a certified birth doula and complete CPR training.
• Fluent in English; bilingual in Spanish (written and verbal).
• Must enroll in Health First Colorado Medicaid.
• Comfortable traveling regularly across assigned counties.

Responsibilities

• Deliver Care Management to meet patient needs via outreach, check-ins, and assessments.
• Coordinate care services and referrals with health plans and care teams.
• Use motivational interviewing and trauma-informed care to engage patients.
• Document and track services in care management portals and meet caseload targets.
• Conduct in-person and virtual community outreach at clinics, WIC sites, and events.
