Job summary

Malama Health is seeking a Community Doula to serve as a Maternity Care Navigator, using our tools to support pregnant and postpartum members. You will bridge providers, community organizations, and patients to ensure smooth care delivery and adherence to care protocols.

You are passionate about maternal health, comfortable with outreach, and eager to learn about care coordination in a startup healthcare tech environment.

Qualifications

• Certified birth doula required with CPR training.
• Experience in community health or care coordination preferred.
• Fluent English; bilingual in Spanish is a plus.

Responsibilities

• Deliver Care Management to meet patient needs via calls, SMS, video, or in-person visits.
• Coordinate care services with health plans and providers to complete referrals.
• Use motivational interviewing and trauma-informed care to engage patients.
• Document and track outreach and care coordination in portals and systems.
• Conduct in-person and virtual outreach at clinics and community sites; staff events as needed.
