Job summary

CareSource is seeking a Maternity Care Coordinator to serve as the primary contact for assessment, planning, and coordination under the Mom and Baby Beginnings program.

The role emphasizes engaging members, performing health and psychosocial surveys with cultural awareness, and coordinating care with physicians and a multidisciplinary team. Strong communication and organizational skills are essential.

Qualifications

• Diploma or Associates in a healthcare related field or equivalent experience.
• Bachelor’s degree in a healthcare related field preferred.
• Minimum of three years of experience in a healthcare field (e.g. discharge planning, case management, care coordination).

Responsibilities

• Telephonically engage and enroll members in Mom and Baby Beginnings program.
• Educate members on the Mom & Baby Beginnings program and resources.
• Conduct initial screening and needs assessments for members.
• Verify eligibility, demographics, and current health status.
• Document care coordination activities and member responses.
• Coordinate care with assigned physicians and external providers.
• Ensure timely and accurate documentation per policies.
• Maintain current documentation in electronic records per compliance guidelines.
• Monitor service delivery and quality, seeking improvements where possible.
• Support internal care coordination activities to improve member outcomes.
