Responsibilities

• Ensure smooth transitions across cardio-obstetric care continuum from prenatal to postpartum.
• Identify and enroll obstetric patients with cardiovascular risk factors into the program.
• Serve as primary contact for patients navigating the multidisciplinary care pathway.
• Counsel patients and families on cardio-obstetric conditions and management.
• Coordinate outreach to rural/remote clinics and travel as scheduled.
• Refer patients to financial counseling, social services, and clinical programs.
• Develop and maintain patient education materials and community outreach events.
• Maintain accurate documentation for monthly DOH billing and contract reporting.
• Participate in quality programs to improve care and efficiency.
