Qualifications

• Bachelor's degree in behavioral health, social work, human services, or related field.
• 3 years of experience in Social Work or another related field.
• Experience in substance use treatment or behavioral health settings.
• Knowledge of recovery-oriented, trauma-informed, and family-centered care.
• Ability to meet documentation, billing, and productivity requirements.

Responsibilities

• Conduct comprehensive biopsychosocial assessments to identify client needs across behavioral health, physical health, housing, employment, legal, and social determinants domains.
• Develop, implement, and update individualized treatment plans in collaboration with clients.
• Provide individual case management and skills training sessions.
• Facilitate group sessions as assigned.
• Coordinate and document referrals, monitor follow-through, and advocate for access to support (medical providers, other behavioral health providers, child welfare, vocational, housing, transportation, community resources).
• Monitor client progress toward treatment goals and document outcomes.
• Participate in multidisciplinary treatment team meetings and transition/discharge planning.
• Provide education related to substance use recovery, parenting, prenatal/postpartum health, and life skills.
• Support client engagement, retention, and adherence to treatment recommendations.
