Job summary

WES Health System is seeking a Case Manager to support pregnant women and families by arranging health care and social services, teaching self-care, and offering emotional support. The role includes maintaining a caseload of about 250 clients and conducting outreach to connect eligible clients with needed services.

Responsibilities include supporting mothers during pregnancy and after birth, promoting positive parenting and well-child care, and linking families to resources while navigating

Qualifications

• Bachelor’s Degree in Social Work, Psychology or related field from an accredited college or university.
• Minimum of one year of case management or counseling experience or counseling experience with children and families preferred.
• Valid Illinois state criminal, child abuse, and FBI history required.
• Proficiency in speaking English and Spanish preferred.
• Reliable transportation required.

Responsibilities

• Provide help to pregnant women and children in obtaining health care and social services.
• Teach women how to care for themselves and their new families.
• Provide emotional support when needed and link women to needed services.
• Maintain a caseload of approximately 250 active clients through outreach and case finding.
