Job summary

You will perform assessments, coordinate care, and link clients to essential community resources to support recovery and stability. This role serves the Gatewayregion, requiring collaboration with multiple community partners and careful documentation of client progress and referrals

Qualifications

• Bachelor’s degree in a social or behavioral science field is required.
• Minimum of one year of case management experience.
• Ability to complete required training within six months of hire.

Responsibilities

• Conduct client outreach, assessments, and individualized service planning.
• Provide education on recovery, treatment options, and support services.
• Coordinate care and make referrals to appropriate agencies and providers.
• Serve as the primary contact for referrals from local health departments.
• Collaborate with courts, DCBS, law enforcement, and shelters as needed.
• Maintain accurate documentation and case records.
• Support clients in navigating complex systems and accessing services.
