Job summary

Advocate Health seeks an experienced diabetes education specialist in Charlotte, NC to provide individualized education for high-risk obstetric patients. You will review CGM data, guide insulin and medication management, and coordinate follow-up care across in-person and telehealth formats.

Required RN licensure or healthcare-related bachelor’s degree with at least two years of clinical experience. Strong communication, teaching skills, and ability to work in a multidisciplinary team are

Responsibilities

• Educate patients with gestational diabetes, type 1 and type 2 diabetes and other pregnancy-related metabolic conditions.
• Review blood glucose logs, CGM reports, and insulin pump downloads to assess glycemic control.
• Coordinate in-person, telehealth, and remote follow-up care.
• Participate in multidisciplinary care planning, quality improvement initiatives and program development activities.
