Job summary

Advocate Health in Charlotte, NC seeks a full-time diabetes education professional to join the Southpark Diabetes Clinic. Responsibilities include leading DSMT and MNT education, tailoring sessions to patient needs, and collaborating with the care team to implement comprehensive care plans.

Licensed RN or Registered Dietitian required; CDCES certification preferred. Will travel 2-3 days per week to additional locations. Travel is expected but compensated within a supportive rewards program.

Qualifications

• Current license or certification to practice as a Registered Nurse (RN) or Registered Dietitian (RD) in the applicable state.
• CDCES preferred; certification must be obtained within 2–3 years if not already held.

Responsibilities

• Provide DSMES per ADA/ADCES standards.
• Assess learning needs, goals and barriers; tailor education.
• Lead individual and group diabetes management education sessions.
• Provide medical nutrition therapy as appropriate.
• Coordinate with care team to implement comprehensive care plans.
• Stay current on diabetes management technologies and guidelines.
• Document and bill for DSMT and MNT services per CMS rules.
