Ingham County Health Department is seeking a Community Health Worker to link underserved communities with health and social resources. The CHW will assist clients with forms, appointments, and referrals, and will help navigate health programs from Medicaid to food assistance.

This role involves home visits, outreach, and coordination with care teams. The position emphasizes cultural competency, effective communication, and a client-centered approach to addressing social determinants of health

Competenze

• High School diploma or equivalent required.
• MiCHWA curriculum post-hiring preferred.
• Experience with social services, public health systems, or maternal child health services preferred.

Mansioni

• Assist clients with filling out forms and coordinating appointments.
• Develop and share resource guides for food, housing, and services.
• Document interactions using client systems and care coordination protocols.
• Assess social determinants of health and provide referrals.
• Assist with Medicaid, SER, and food stamps processes and related tasks.
• Provide Medicaid outreach and education to clients and the public.
