Job summary

University of Maryland Faculty Physicians Inc. is seeking a Community Health Worker to conduct home visits and community outreach for individuals and families, often in shelters, to support coping with a new diagnosis.

Under supervision, you will provide basic social interventions and connect clients with resources; requires CHW certification and experience with diverse patient populations.

Qualifications

• High School Diploma or GED required.
• Certification as Community Health Worker from an accredited program.
• Minimum 2 years of experience as CHW/outreach worker with diverse populations, preferably in a medical setting.

Responsibilities

• Visit individuals and families in homes, shelters or similar places to provide basic community outreach services and assist with social interventions for coping with a new diagnosis.
