Job description

The purpose of this position is to support the delivery and advancement of Maternal and Child Health (MCH) and immunization programs throughout Delaware County. The position provides education, care coordination, outreach, and resources to priority populations while promoting access to preventive health services. The role also actively contributes to program development, implementation, and continuous improvement to address current and emerging maternal-child health and immunization needs within the community.

• Staff supervision and oversight including assigning, reviewing, evaluating and monitoring productivity of the work of the MCH Staff.
• Day-to-day supervision, monitoring staff productivity, ensuring appropriate staffing levels at all times, hiring and training new staff, performance planning and assessment, planning and convening staff meetings.
• Providing clinical expertise and guidance to ensure quality Maternal Child Health services are delivered.
• Working with Personal Health Administrator on program planning, policy development, and budget planning.
• Serves as a lead program administrator for multiple required and suggested activities as defined in the workplan for the DCHD-funded Immunizations and Vaccines for Children (VFC) grant.
• Implement vaccine management and accountability policies as defined through the annual program grant.
• Implements policy and plan for routine use of 317-funded vaccine only for individuals who are not fully insured with respect to vaccine. Plan must adhere to guidance set forth in "Use of Vaccine Purchased with 317 Funds".
• Maintains an inventory of provider education for CDC compliance. Ensures that all staff in the program keep this inventory current. Uses this inventory to plan for and implement future education efforts for the program.
• Designs and establishes quality assurance protocols to increase Vaccines for Children (VFC) Program efficiency.
• Manages the Title V Block grant by ensuring that deliverables are met by the end of each reporting year.
• Oversees VFC providers compliance reviews and IQIP program and serves as back-up to IQIP specialist.
• Perform other duties, tasks, and special projects, as required
• Answer after-hours call according to protocol.
• Occasional evening and weekend hours are required for special events.
• Participate in DCHD committees, workgroups, quality improvement initiatives, accreditation activities, emergency preparedness activities, and special projects as assigned to support departmental goals and organizational priorities.
• Perform other duties as assigned.

• Bachelor of Science in Nursing (BSN) from an accredited college or university.
• Current, unrestricted license as a Registered Nurse (RN) issued by the Pennsylvania State Board of Nursing.
• Current CPR/First Aid certification, or ability to obtain within a designated period following hire.
• Knowledge of maternal and child health, immunization practices, and the Vaccines for Children (VFC) Program.
• Knowledge of public health nursing principles and practices, including health promotion, disease prevention, community-based care, and health education.
• Demonstrated ability to provide health education, care coordination, and resource navigation to individuals and families.
• Strong interpersonal, verbal, and written communication skills, with the ability to communicate effectively with individuals from diverse backgrounds.
• Demonstrated critical-thinking, problem-solving, and sound decision-making skills.
• Ability to develop and maintain collaborative relationships with healthcare providers, community organizations, schools, government agencies, and other community partners.
• Ability to appropriately handle and maintain confidential and protected health information in accordance with applicable laws, policies, and procedures.
• Ability to follow clinical protocols, policies, procedures, and established public health guidelines.
• Ability to effectively manage multiple priorities and adapt to changing programmatic and community health needs.
• Proficiency with standard computer applications and the ability to learn electronic health records, immunization registries, and other public health information systems.

• Valid driver’s license and ability to meet applicable background and pre-employment requirements.

Preferred Qualifications & Experience

• Master of Science in Nursing (MSN), Master of Public Health (MPH), or other relevant advanced degree from an accredited college or university.
• Professional nursing experience in maternal and child health, pediatrics, immunizations, community health, or public health nursing.
• Experience administering vaccines and working with the Vaccines for Children (VFC) Program or other publicly funded immunization programs.
• Experience providing community-based health education, outreach, case management, or care coordination to maternal, infant, child, and adolescent populations.
• Experience participating in program development, implementation, evaluation, or quality-improvement initiatives.
• Experience working collaboratively with community-based organizations, healthcare systems, schools, and other public health partners.
• Experience working with diverse, underserved, or historically marginalized populations.
• Pediatric nursing experience is strongly preferred.
• Bilingual proficiency, particularly English/Spanish, is preferred.

Physical Demands

• Ability to perform duties in clinical, office, community, and residential/home environments.
• Ability to sit, stand, walk, bend, reach, and move throughout the workday.
• Ability to travel to homes, schools, community sites, clinics, and other locations throughout Delaware County.
• Ability to carry and transport nursing, screening, educational, and outreach supplies and equipment, generally up to 25 pounds.
• Ability to perform clinical activities requiring sufficient manual dexterity, including specimen collection, point-of-care testing, vaccinations, blood pressure measurements, and other nursing procedures within the individual's scope of practice.
• Ability to use computers, electronic health records, telephones, and other standard office and clinical equipment for extended periods.
• Ability to communicate effectively in person, electronically, and by telephone.
• May occasionally be exposed to communicable diseases, blood or other bodily fluids, environmental hazards, and adverse weather conditions while performing field-based duties.
• Must be able to use required personal protective equipment and follow infection prevention, occupational health, and safety protocols.

The selected candidate will be required to obtain, at their own expense, 3 forms of clearance, including PA State Police Criminal History Record Check; PA Child Abuse History; and FBI Criminal History Background Check including finger printing prior to employment.

Delaware County Health Department requires all employees to have received at least one COVID-19 vaccination in the past 3 years. Proof of vaccination must be submitted before a candidate’s start date, if hired.
