Job summary

Carolina Family Health Centers, Inc. seeks a Medication Assisted Treatment Program Manager for Freedom Hill Community Health Center in Tarboro, NC. The role leads the MAT program across sites and provides individual psychotherapy to patients with substance use disorders.

Qualifications

• Master’s Degree in Social Work from an accredited university.
• Fully licensed as a Licensed Clinical Social Worker (LCSW) in North Carolina.
• LCAS licensure preferred in addition to primary licensure as an LCSW.
• BLS Certification.
• Valid NC Driver License.

Responsibilities

• Oversee and coordinate the MAT program across all sites.
• Coordinate MAT services across service delivery sites to promote an integrated model of care.
• Develop, implement, evaluate, and continually improve MAT workflows and procedures.
• Monitor MAT schedules for providers and LCSWs; plan coverage for absences, meetings, holidays, and weather closures.
• Contribute to reporting, data collection, and quality assurance for substance use grants.
• Maintain MAT policies, procedures, and consent forms across the organization.
• Co-chair MAT Case Conference meetings and follow up on recommendations.
• Establish and report MAT program performance measures with leadership.

Education

Master’s Degree in Social Work

Licensed Clinical Social Worker (LCSW) in North Carolina
