Sun River Health is seeking a full-time Licensed Practical Nurse to support the Amityville site, splitting 2 days per week in the Genesis department and 3 days per week in the Medical department. The role involves delivering nursing care under medical staff direction and adhering to center policies.

Qualified applicants will hold a New York State LPN license, a Vocational/Technical Nursing diploma, CPR and Infection Control certifications, and are committed to patient education and quality care

Qualifications

• Minimum: Vocational/Technical School/Diploma Program – Nursing.
• Valid New York State LPN License.
• CPR Certification.
• Infection Control Certification.

Responsibilities

• Interview and maintain patient’s medical record, including nursing observations, health history, medications, vital signs, vision/hearing tests, and laboratory results.
• Perform in-house laboratory tests, collect specimens for outside labs, and conduct quality control in accordance with protocols.
• Complete and update all assigned tasks and logs.
• Initiate follow-up as directed by provider (e.g., encounter, labs, close non-billables).
• Provide health teaching as indicated by physician or patient inquiry. Refer to RN as needed.

Sun River Health is a Federally Qualified, Non‑Profit Health Center serving communities in Suffolk, Rockland, Orange, Dutchess, Ulster, Sullivan, Columbia and Westchester County. The center is seeking a full‑time Licensed Practical Nurse to support the Amityville site, working 2 days per week in the Genesis department and 3 days per week in the Medical department.

Position Summary

A Licensed Practical Nurse renders nursing care within the center or authorized community site, under direction of the medical staff and in line with the center’s policies. The role includes performing duties in compliance with system and job‑specific standards, and additional tasks as assigned.

Pay Range

$29.00 – $33.02 per hour

Sign‑On Bonus

$2,700 Sign‑On Bonus for eligible external applicants – Terms and conditions apply.

Essential Duties And Responsibilities
