Job summary
Crossroads Treatment Centers in Virginia is seeking a Substance Abuse Counselor to join our outpatient MAT program in Winchester. You will provide individual counseling to highly complex patients, perform clinical assessments, design and update treatment plans, and collaborate with nurses and other providers to coordinate care.
Responsibilities include managing a caseload (40-55 patients), conducting urine drug screens, and supporting discharge planning.
Qualifications
- Experience working with underserved populations in a clinical setting.
- Lived addiction experience is a plus.
- Licensed to practice in Virginia or eligible for licensure.
Responsibilities
- Provides individual counseling to highly complex patients.
- Conducts clinical assessments to determine treatment needs.
- Designs and updates treatment plans, including care coordination, referrals, and discharge planning.
- Manages assigned caseload and complies with clinical treatment requirements.
- Collaborates with nurses and providers to deliver holistic treatment services.
- Facilitates urine drug screening for patients.
