The role focuses on coordinating medication-assisted treatment for incarcerated individuals and ensuring a recovery-oriented continuum of care within a secure setting.

The LPN must be licensed in Delaware and will work with the MAT Case Manager to oversee patient care, medication management, and collaboration with community providers.

Qualifikationen

• Must have and maintain current licensure as a Licensed Practical Nurse in Delaware.

• Initial and ongoing SUD assessment and evaluation
