VitalCore Health Strategies, (VCHS), an industry leader in Correctional Health Care, has openings for a MAT Coordinator Licensed Practical Nurse (LPN) at Sussex Correctional Institution in Georgetown, DE!

The MAT Nurse will join a team to oversee patient care for incarcerated individuals receiving MAT and deliver a recovery‑oriented system of care. LPN licensure in Delaware is required and you’ll work on-site in the facility.

Qualifications

• Must hold and maintain a current Delaware LPN license.
• Experience with medication inventory and MAT processes is preferred.
• Ability to assess and coordinate care for individuals with substance use disorders.

Responsibilities

• The MAT LPN works with the MAT Case Manager to manage all patient care for incarcerated individuals on MAT.
• Coordinate care across treatment services, ensure accurate medication dispensing, and participate in re-entry planning.
• Engage with community providers and participate in required trainings.

MAT COORDINATOR/ LICENSED PRACTICAL NURSE (RN) POSITION SUMMARY:

The Medication Assisted Treatment Licensed Practical Nurse works as part of a team with the MAT Case Manger to manage all aspects of patient care for incarcerated individuals receiving Medication Assisted Treatment for Opioid Use Disorder and deliver a recovery‑oriented system of care.

MAT COORDINATOR / LICENSED PRACTICAL NURSE (LPN) MINIMUM REQUIREMENTS:

• Must have and maintain current licensure as a Licensed Practical Nurse in Delaware.

• Medication Inventory Management
• Initial and ongoing SUD (substance use disorder) assessment and evaluation
• Patient Care Coordination
• Collaborate with community treatment providers and social support services
