Job summary

VitalCore Health Strategies is seeking a MAT Coordinator / Licensed Practical Nurse (LPN) at Sussex Correctional Institution in Georgetown, DE. The role focuses on coordinating medication-assisted treatment for incarcerated individuals and ensuring a recovery-oriented continuum of care within a secure setting.

The LPN must be licensed in Delaware and will work with the MAT Case Manager to oversee patient care, medication management, and collaboration with community providers.

Qualifications

• Must have and maintain current licensure as a Licensed Practical Nurse in Delaware.
• Medication Inventory Management
• Initial and ongoing SUD assessment and evaluation
• Patient Care Coordination
• Collaborate with community treatment providers and social support services
• Health Promotion
• Comprehensive transitional care and re-entry planning
• Attend meetings and trainings as required
• Dispense MAT Medications

MAT COORDINATOR / LICENSED PRACTICAL NURSE (RN) POSITION SUMMARY:

The Medication Assisted Treatment Licensed Practical Nurse works as part of a team with the MAT Case Manger to manage all aspects of patient care for incarcerated individuals receiving Medication Assisted Treatment for Opioid Use Disorder and deliver a recovery-oriented system of care.

MAT COORDINATOR / LICENSED PRACTICAL NURSE (LPN) MINIMUM REQUIREMENTS:

• Must have and maintain current licensure as a Licensed Practical Nurse in Delaware.

• Medication Inventory Management

• Initial and ongoing SUD (substance use disorder) assessment and evaluation
