Job summary

River Valley Behavioral Health seeks an MAT Case Manager to coordinate care for patients on medication-assisted treatment and support long-term recovery. You will liaise with patients, prescribers, nursing staff, and community resources to ensure continuity of care and effective treatment.

You will conduct intake assessments, monitor progress, provide counseling and referrals, and maintain compliant documentation while advocating for patients in a harm-reduction framework.

Qualifications

• Bachelor's in Human Services or related field, or degree with healthcare experience.
• Background in social work, counseling, psychology or related behavioral health.
• Knowledge of substance use disorders and evidence-based approaches.
• Strong organizational and case management skills.
• Ability to build rapport and maintain professional boundaries.
• Valid driver's license and reliable transportation.

Responsibilities

• Coordinate intake assessments and develop individualized treatment plans.
• Monitor progress, adherence to meds, and treatment goals.
• Provide counseling, education, and referrals to supportive services.
• Coordinate care with physicians, nurses, therapists, and outside providers.
• Maintain accurate, timely documentation per standards.
