Job summary

Qualifications

• CARES/CPS-AD certification required
• High school diploma or equivalent; Associate’s or Bachelor’s degree in healthcare, social work, or related field preferred
• Experience in case management or addiction services
• Strong understanding of Medication-Assisted Treatment principles and addiction recovery processes
• Excellent communication and interpersonal skills to engage with diverse populations
• Proficiency with electronic health records (EHR) systems and data entry

Responsibilities

• Serve as a primary point of contact for patients seeking information and access to Medication-Assisted Treatment programs.
• Collaborate with healthcare providers, social workers, and community organizations to support patient engagement and retention in treatment.
• Provide follow up and appointment reminders for MAT patients.
• Track and document patient interactions and outcomes to support program evaluation and continuous improvement.
• Provide education and outreach to patients and families about the benefits and processes of MAT.
• Assist in identifying and addressing barriers to treatment access, including insurance, transportation, and stigma-related challenges.
