Job summary

Codman Boston is seeking a Clinical Care Coordinator for the OBAT program to support MAT delivery, care management, and logistics. You will work with an interdisciplinary team under supervision of NCM and report to the OBAT supervisor.

The role emphasizes coordinating intakes, scheduling, vital signs, drug screening, pharmacy assistance, prior authorizations, and referral coordination to ensure access to services. A health-focused background and strong collaborative skills are essential.

Qualifications

• Minimum: High school diploma or GED.
• Preference for Bachelor's degree in health-related field.
• Graduate of an accredited Medical Assistant program.

Responsibilities

• Support OBAT clinic operations including vital signs and screenings.
• Coordinate intakes and scheduling; assist with drug screening.
• Assist with prior authorizations and pharmacy support.
• Maintain OBAT Registry and coordinate care with NCM.
• Attend weekly case conferences and coordinate patient care.
• Provide transportation assistance and case management for patients.
• Assist with reporting and tracking measures.
