Job summary

Fora Health is seeking a Medical Programs Case Coordinator- MAT to support the medical team across MAT, residential, withdrawal management, and psychiatric services. The role focuses on coordinating patient care, enabling smooth transitions, and supporting clinical operations.

You will work with nurses, counselors, peer mentors, and administrative staff to ensure high-quality care and continuity. A Bachelor’s degree is preferred, with at least three years in healthcare or behavioral health.

Qualifications

• Bachelor’s degree preferred in a related field or equivalent experience.
• Minimum 3 years of relevant experience in healthcare or behavioral health, ideally with substance use disorder and underserved populations.

Responsibilities

• Coordinate patient appointments and transitions between programs; provide referrals and follow-ups.
• Assist with medication management, audits, and coordination with pharmacy partners.
• Serve as liaison between medical, residential, and administrative teams to ensure continuity of care.
• Maintain accurate records and ensure compliance with confidentiality and ethics policies.
