Job summary

New Horizons Behavioral Health is seeking a Program Coordinator for the Medication Assisted Treatment (MAT) program in Columbus, GA. This full-time role runs Monday through Friday, 8:00 a.m. to 5:00 p.m., with a one-hour lunch break, under general supervision.

The candidate will manage referrals, schedule intake appointments, coordinate with MAT teams, complete PHI releases, track progress in WITS, and support clients with treatment access and resources to maintain sobriety.

Qualifications

• High School Diploma or GED plus 3–5 years in behavioral health or Bachelor’s in social sciences with 1 year full-time experience in behavioral health.
• If candidate has a Bachelor's, it MUST be in the named fields to be MHC eligible.
• Candidate MUST have 2+ years in recovery personally; lived experience supports services.

Responsibilities

• Receive and follow up on referrals.
• Make intake appointments.
• Refer after intake to MAT-SOR program.
• Complete documentation and PHI releases.
• Contact MAT individuals weekly with progress notes in WITS.
• Assist with MAT needs, doctor visits, medications, and treatment access.
• Coordinate with provider and nurse on concerns.
• Attend MAT progress meetings.
• Procure and sign out Narcan.
• Lead evidence-based groups for individuals with substance use disorders.
• Document daily notes and provide community resources for sobriety.
• Help increase coping and basic living skills.

Education

High School Diploma or GED

Bachelor’s degree in social sciences (Social work, psychology, sociology, counseling, or related discipline)

Program:Medication Assisted Treatment (MAT)

Job Type:Full-Time (40 hours per week)

Days:Monday through Friday, 8am to 5pm, with a 1-hour lunch break

Works with the following population:Males and females, 18 years of age or older,with a mental health disorder and/oraddictive diseases.

Salary: $33,072 per year

Under general supervision, the candidate will do the following:

• Receive and follow up on referrals.
• Make appointments for intake.
  • After intake, complete referral to New Horizons Behavioral Health MAT-SOR Program.
• Complete Medication Pharmacy Grant Rx Payment Voucher to Genoa Healthcare.
• Complete DHP Authorization for Release of Protected Health Information (PHI).
• Complete Midtown Recovery Release of PHI
• Contact MAT individuals weekly with documentation into WITS system
• Assist with any needs for MAT, doctor's appointments, getting medication, and assistance with receiving treatment.
• Coordinate with provider and nurse to address any concerns they may have.
• Attend meetings for MAT to update on progress.
• Procure and sign out Narcan.
• Conduct evidence-based group with individuals who have been identified with a substance abuse disorder.
• Document daily notes on progress in groups and provide resources in the surrounding community that aid in maintaining sobriety.
• Aid in increasing coping and basic living skills.

Documentation and Reporting

Maintain accurate confidential records of client interactions and progress. Prepare reports and documentation in compliance with organizational and regulatory requirements.

Collaboration

Collaborate with a multidisciplinary team, including mental health professionals, to ensure holistic care for clients. Attend team meetings and case conferences to contribute a peer perspective to the overall care plan.

Minimum Qualifications:

High School Diploma or GED, with 3 to5 years of experience in behavioral health -or- completion of a bachelor's degree in the social sciences (Socialwork, psychology, sociology, counseling, or a related discipline) with 1 year of full-time experience working with individuals with mental health disorders and/or addictive diseases. If candidate has a Bachelors, it MUST be in theabove-named fields to be MHC eligible. The candidate MUSThave 2 years or more in recovery himself / herself, as lived experience will be used to support individuals in services.

Preferred Qualifications:

A bachelor's degree and experience in working with individuals with mental disorders and/or addictive diseases is preferred.
