  • Collaborate with healthcare providers, social workers, and community organizations to support patient engagement and retention in treatment.
  • Provide follow up and appointment reminders for MAT patients.
  • Track and document patient interactions and outcomes to support program evaluation and continuous improvement.
  • Provide education and outreach to patients and families about the benefits and processes of MAT.
  • Assist in identifying and addressing barriers to treatment access, including insurance, transportation, and stigma-related challenges.

About the Role:

The MAT Connections Specialist plays a critical role in bridging the gap between patients seeking Medication-Assisted Treatment (MAT) and Ascensa Health. This position is focused on facilitating access to treatment resources, ensuring patients receive timely and appropriate care to support their recovery journey. The specialist will work closely with healthcare professionals, community organizations, and patients to coordinate services and provide education about MAT options.

Minimum Qualifications:

  • CARES/ CPS-AD certification required
  • High school diploma or equivalent; Associate’s or Bachelor’s degree in healthcare, social work, or related field preferred.
  • Experience working in case management or addiction services.
  • Strong understanding of Medication-Assisted Treatment principles and addiction recovery processes.
  • Excellent communication and interpersonal skills to effectively engage with diverse populations.
  • Proficiency with electronic health records (EHR) systems and basic data entry.

Preferred Qualifications:

  • Bachelor’s degree in public health, social work, psychology, or a related discipline.
