Qualifications

• REQUIRED EDUCATION: Master’s degree in behavioral health related field preferred.
• REQUIRED EXPERIENCE: At least three years of behavioral health experience. Prior social work, case management or family therapy experience preferred.
• LICENSURE/CERTIFICATION: LPC-A, LPC, LMSW, or LCSW preferred.

Responsibilities

• Provide navigation for patient and families by addressing any concerns and providing insight to the continuum of care.
• Assist patient and families in connecting with necessary supportive after-care services, psychosocial care, and social work.
• Maintain regular communication and updates with and to the patient’s providers.
• Refer to internal or external case management services when other issues are identified (i.e. hunger/homelessness issues, domestic violence issues, etc.)
• Provide advocacy, patient education and support in accessing community-based and hospital-based programs.
