Peachford Hospital is seeking a full-time, day shift Master’s prepared Case Manager/Social Worker (MSW) to join an interdisciplinary treatment team in the Atlanta area. The role focuses on psychosocial assessment, discharge planning, and connection to community resources to support patients across admissions to discharge.

A license-eligible MSW is required; inpatient/outpatient hospital experience is preferred.

Qualifications

• License-eligible master’s degree in a related clinical area.
• Prior inpatient/outpatient hospital experience preferred.

Responsibilities

• Conduct psychosocial assessments documenting patient history and treatment focus.
• Provide individual, group and family solutions-focused interventions.
• Facilitate process and didactic groups.
• Develop individualized discharge plans with input from the treatment team and family.
• Identify family or community resources such as group homes and treatment providers.
