Job Title: Master Social Worker (MSW)

Location: New York

Hour: Per diem

Hourly Rate: $30-35 per hour

Position Overview: We are seeking a dedicated MSW to work with individuals to develop and implement person-centered plans supporting their goals, independence, and community integration.

Responsibilities:

1. Conduct initial assessments to understand individual strengths, needs, and goals.
2. Develop person-centered plans to support community engagement and quality of life.
3. Provide counseling for mental health, behavioral, or other issues.
4. Offer life skills training, including social, communication, and problem-solving skills.
5. Connect clients with community resources and activities.
6. Encourage community participation and relationships.
7. Facilitate access to vocational training and supported employment.
8. Advocate for clients to access necessary services and programs.
9. Support families and caregivers with resources and information.
10. Develop crisis intervention strategies.
11. Refer clients to specialized services as needed.
12. Maintain records of client progress and service plans.

Qualifications:

• Bachelor's degree in social work, psychology, or related field.
• Two years of experience counseling seniors or individuals with disabilities and their families.
• Knowledge of NHTD/TBI programs and regulations.
• Strong communication and interpersonal skills.
• Ability to work independently and in a team.
• Proficiency in electronic health record systems.
• Valid driver's license and reliable transportation.
