Job Details:

Title: Master Social Worker (MSW)

Location: New York

Type: Per diem

Hourly Rate: $30-35 per hour

Position Overview:

Seeking a dedicated MSW to work directly with individuals, developing and implementing person-centered plans that support their goals, promote independence, and community integration.

Responsibilities:

• Conduct initial assessments to understand individual strengths, needs, and goals.
• Develop person-centered plans to support community engagement and quality of life.
• Provide counseling services for mental health, behavioral challenges, and community involvement.
• Offer life skills training, including social, communication, and problem-solving skills.
• Connect clients with community resources, programs, and activities.
• Encourage participation in community events and foster relationships.
• Facilitate access to vocational training and supported employment.
• Advocate for clients to access necessary services and support.
• Support families and caregivers with information and resources.
• Develop crisis intervention strategies as needed.
• Refer clients to specialized services when necessary.
• Maintain detailed records of client progress and service plans to ensure compliance.

Qualifications:

• Bachelor's degree in social work, psychology, or related field.
• Minimum two years' experience in counseling seniors or individuals with disabilities and their families, especially related to adjustment and community integration.
• Knowledge of NHTD/TBI programs and regulations.
• Strong communication and interpersonal skills.
• Ability to work independently and as part of a team.
• Proficiency in electronic health record systems; valid driver's license and reliable transportation.
