Job Title: Master Social Worker (MSW)

Location: New York

Hour: Per diem

Hourly Rate: $30-35 per hour

Position Overview: We are seeking a dedicated MSW to work directly with individuals to develop and implement person-centered plans supporting their goals and promoting independence and community integration.

Responsibilities:

  1. Conduct initial assessments to understand individual strengths, needs, and goals for community integration.
  2. Develop person-centered plans outlining support needed to enhance community engagement and quality of life.
  3. Provide counseling to manage mental health, behavioral challenges, or other issues affecting community involvement.
  4. Offer life skills training, including social, communication, and problem-solving skills.
  5. Connect clients with community resources such as vocational programs, recreational activities, and social groups.
  6. Encourage participation in community events, fostering relationships and independence.
  7. Facilitate access to vocational training or supported employment.
  8. Advocate for clients to access necessary services and programs.
  9. Support families and caregivers with information and resources.
  10. Develop crisis intervention strategies when needed.
  11. Refer clients to specialized services like mental health treatment or rehabilitation.
  12. Maintain detailed records of client progress and service plans to ensure compliance with regulations.

Qualifications:

  • Bachelor's degree in social work, psychology, or related field.
  • At least 2 years of experience providing counseling to seniors or individuals with disabilities and their families.
  • Knowledge of NHTD/TBI program regulations.
  • Strong communication and interpersonal skills.
  • Ability to work independently and in a team.
  • Proficiency in electronic health record systems.
  • Valid driver's license and reliable transportation.
