Job summary

Nurse Practitioner On Call (NPOC) is seeking an experienced Community Liaison to expand our client base and establish partnerships with Maryland long-term care facilities. You will identify opportunities, build relationships with decision-makers, and guide opportunities from outreach through proposals to service launch.

The ideal candidate has direct Maryland long-term care market experience, established relationships, and a proven ability to convert outreach into lasting partnerships.

Qualifications

• Minimum 3 years in healthcare business development or community liaison roles in Maryland.
• Experience with long-term care facilities and decision-makers.
• Proven ability to generate referrals and develop partnerships.
• Existing relationships within Maryland’s long-term care or healthcare referral community.

Responsibilities

• Develop Maryland facility partnerships with nursing homes and long-term care providers.
• Grow referrals through community outreach, presentations, and networking.
• Own the growth process with Maryland outreach and a business development plan.
• Maintain records of prospects, meetings, proposals, and contract status.
• Coordinate with intake and clinical teams for timely referrals and onboarding.
