Job summary

Easter Seals Tennessee Inc seeks a MAPS Lead to help individuals with intellectual and developmental disabilities set goals, learn skills, and access resources for independence. The role focuses on developing plans, leveraging technology, and coordinating with families and service providers to advance milestones.

The candidate will travel monthly for face-to-face meetings, document progress, and maintain strong communication with the individual and family, ensuring safety and quality services

Qualifications

• High School Diploma or GED is required.
• Experienced user of technology and digital tools.
• Ability to organize, prioritize tasks and manage time effectively.
• Strong verbal and written communication skills for reports and records.

Responsibilities

• Meet with individuals and families to set goals and milestones for yearly service plans.
• Create and update a Virtual Community Resource Map detailing jobs, housing, transportation, and activities.
• Teach the use of technology to access the VCRM and support independence.
• Support individuals in achieving milestones including independent living, work, and travel.
• Coordinate with families, coordinators, and vendors to access enabling technology and resources.
• Document progress with milestones and submit monthly service notes on time.
• Attend monthly provider meetings, in person or virtually, and ensure safety and compliance.
