Job summary

Community Options, Inc. is seeking a Full-Time Medicaid Alternative Pathways (MAPs) Coordinator in Rutherford, TN to lead the MAPs program, coaching participants and their networks to foster independent living and community integration using technology and virtual mapping.

The role involves managing a MAPs caseload, developing a VCRM, and ensuring timely documentation. Requires at least a high school diploma and 2 years in IDD or related human services.

Qualifications

• High school diploma or equivalent.
• Minimum of two years of full-time experience in the IDD or related human services field.
• Strong ability to communicate effectively with individuals at all levels.
• Ability to establish supportive relationships with individuals served.
• Proficiency with technology and digital tools.
• Clear written and spoken English.

Responsibilities

• Manage a caseload of MAPs participants.
• Collaborate to develop a Virtual Community Resource Map (VCRM).
• Train participants and support networks on enabling technologies.
• Coach on independent living skills, job readiness, transportation, and health management.
• Ensure required documentation is completed accurately and timely.
• Attend required trainings and cooperate during inspections.
