Job summary

The position requires 2+ years of direct care experience, MAP certification within 6 months, and a valid MA driver’s license. You will document progress in EMR and support residents in independent living and crisis management.

Qualifications

• Two years of direct care experience with individuals with mental illness or disabilities.
• Current valid driver’s license (Class D).
• MAP certification within 6 months of entering the position.
• Bilingual or multilingual abilities preferred.
• Experience with DMH regulations and program procedures.

Responsibilities

• Advocate and provide direct one-to-one support to implement the Community Service Plan.
• Maintain EMR documentation of progress and ensure compliance with DMH procedures.
• Ensure safe medication administration under MAP policies.
• Assist residents in developing independent living skills.
• Coordinate recreational, educational, health, and fitness activities with staff and community.
