Job summary

NorthStar Behavioral Health seeks an LPN/LVN for the Medication Assisted Treatment Program. The role involves daily assessment and dosing of MAT clients, coordinating care, and acting as the primary contact for clients presenting for care.

The ideal candidate holds an LPN/LVN license in Ohio, with knowledge of safe nursing practices and the ability to apply essential skills to client care. Experience in chemical dependency is a plus.

Qualifications

• LPN/LVN diploma, associate degree, or equivalent from an accredited school.
• Knowledge of safe nursing practices and application to client care.
• LPN license in the State of Ohio; chemical dependency experience a plus.

Responsibilities

• Dosing of MAT program clients.
• Assess dose changes and follow protocol.
• Transcribe and implement physician orders accurately.
• Document and communicate with counseling, administrative, and medical staff on incidents/concerns.
• Perform daily methadone tally and counts.
• Coordinate care between clinic and hospitals/providers.
• Participate in interdisciplinary team meetings.
• Pursue annual professional goals and continuing education.
• Attend mandatory trainings and department meetings.
• Other duties as assigned.
