Job summary

Thecentersohio seeks a licensed Practical Nurse to provide patient-centered care in Walk-in MAT clinics. You will support medication treatment, patient engagement, and ongoing monitoring in a team-based setting.

Reporting to the Manager of Nursing, you will participate in protocol-driven care, patient education, and coordination with RNs and providers to support treatment adherence and recovery. Must maintain Ohio LPN license and complete CPR and crisis intervention training within 60 days.

Qualifications

• Active, unencumbered LPN license in Ohio.
• CPR certification required.
• Nonviolent crisis intervention training within 60 days of hire.

Responsibilities

• Conduct nursing visits in collaboration with the WinMAT provider and care team.
• Support medication treatment and adherence discussions with patients.
• Document vital signs, substance use patterns, withdrawal and psychiatric symptoms.
• Provide patient education on MAT and harm reduction strategies.
• Administer medications per order and coordinate refills/PA with pharmacies.
• Participate in daily clinical workflows and escalation when needed.
