North Star is seeking a Licensed Practical Nurse (LPN) for the Medication Assisted Treatment Program in Cincinnati, Ohio. This full-time/regular position involves daily dosing, client assessments, and serving as the primary daily contact for clients.

You will collaborate with counseling, administrative, and medical staff, ensure accurate medication orders are followed, and help maintain methadone counts while participating in team meetings and continuing education.

Qualifications

  • Diploma/Associates or LPN/LVN from an accredited school.
  • LPN licensed in the State of Ohio.
  • Experience and/or Certification in Chemical Dependency is a plus.

Responsibilities

  • Dosing MAT program clients.
  • Assesses clients for dose changes and follows orders.
  • Documents and communicates with counseling, administrative, and medical staff on client incidents/concerns.
  • Performs daily methadone tally and counts.
  • Coordinates care between the clinic and other health care providers; acts as client advocate.

  • Maintains ongoing professional development and attends mandatory trainings.

Salary Range: $26.44 To $32.70 Hourly

JOB TITLE: MAP LPN

DEPARTMENT: Medication Assisted Treatment Program

REPORTING TO: Director, MAP Clinic

Position Information: LPN/LVNs perform the daily assessment and dosing of clients participating in In the Medication Assisted Treatment Program. The LPN/LVN is a member of the treatment team and the primary contact daily for clients presenting for care. LPNs/LVNs must comply with all rules, regulations and ethical standards set forth by regulatory agencies.

Essential Duties and Responsibilities:

  • Dosing of Medication Assisted Treatment Program clients.
  • Assesses clients for dose changes and follows orders according to protocol.
  • Ensures accurate transcription and implementation of physician orders.
  • Documents and communicates with counseling, administrative, and medical staff on any client incidents and/or concerns.
  • As assigned, performs daily methadone tally, assuming accountability for an accurate methadone and other medication counts.
  • Coordinates care between the clinic and various hospital or health care providers, serving as a client advocate as necessary.
  • Actively participates in interdisciplinary team meetings and provides relevant client information.
  • Develops and strives to achieve annual individual professional goals.
  • Maintains professional competence through participating in continuing education and other appropriate learning experiences.
  • Attends all mandatory in-service training, regular departmental staff meetings and other meetings as required.
  • Other duties as assigned.

Supervisory Responsibilities [if applicable]: None.

Qualifications:

  • Diploma, Associates or Licensed Practical Nurse (LPN)/ Licensed Vocational Nurse (LVN) from an accredited school;
  • Knowledge of safe nursing practices and the ability to apply the necessary skills and techniques related to +client care;
  • LPN Licensed in the State of Ohio, Experience and/or Certification in Chemical Dependency is a plus.

  • Ability to define problems, collect data, establish facts, and draw valid conclusions.

Physical Requirements:

  • The employee is frequently required to walk, balance, stoop, kneel, and/or crouch. (The employee must occasionally lift and/or move up to 15 pounds).
  • Specific vision abilities required by this job include close vision, distance vision, peripheral vision, depth perception, and ability to adjust focus. Keyboard data entry required.
