Legacy Community Health in Houston seeks a Ryan White Medical Case Manager to deliver psychosocial services to HIV+ patients under Legacy providers. You will link patients to care, support adherence, and provide compassionate case management and advocacy.
The role requires coordination with medical and social services, maintaining EHRS records, and traveling between Legacy locations as needed to ensure comprehensive patient support. Bilingual in English/Spanish preferred.
