Manager of Community Access Programs

About the Role

Are you an operations leader who thrives on keeping multiple programs running smoothly — and making sure patients can get the care they need, when they need it? Thundermist Health Center is looking for a Manager of Community Access Programs to oversee the day-to-day operations of Convenient Care, Telephone Triage, and school-based health programs. You’ll coordinate staffing, manage workflows, monitor access performance, and serve as the operational backbone for some of Thundermist’s most patient-facing programs. If you’re energized by multi-site coordination, fast-paced environments, and health equity, this role is built for you.

What You’ll Do

Convenient Care & Extended Access Operations

  • Manage daily urgent care and walk-in access operations across assigned sites, ensuring consistent, high-quality, patient-centered care delivery
  • Coordinate staffing, scheduling, clinical workflows, and performance expectations for urgent care programs, including evenings, weekends, and holidays
  • Support nursing and MA workforce management including onboarding, schedule coordination, payroll review, PTO/FMLA tracking, coaching, and performance follow-up
  • Serve as the operational point of contact during assigned weekend, evening, and holiday coverage
  • Track access metrics including wait times, visit volume, patient flow, and patient experience data; recommend improvements based on trends
  • Support daily operations of the remote Telephone Triage program including staffing coordination, workflow management, and service level monitoring

School-Based Health Programs

  • Manage daily operations for school-based health programs, supporting consistent service delivery, staffing coverage, documentation, and compliance
  • Maintain effective working relationships with school partners to support program operations and timely issue resolution
  • Oversee scheduling, clinical protocol implementation, documentation standards, and quality measure tracking for school-based health services
  • Identify operational barriers and service gaps and elevate recommendations for improvement to the Director

Cross-Site Coordination & Community Partnerships

  • Coordinate with site Executive Directors, Medical Directors, and operational leaders on workflow issues, staffing needs, and barriers affecting patient access
  • Work with front desk teams to ensure registration processes support timely, seamless patient access; resolve scheduling gaps, insurance verification delays, and intake inefficiencies
  • Maintain day-to-day relationships with community partners, schools, social service agencies, and public health entities
  • Provide direct supervision and support to assigned staff, reinforcing expectations for service quality, teamwork, and accountability
  • Support hiring, onboarding, coaching, performance feedback, and professional development for staff within assigned programs
What We’re Looking For

You’re an adaptable operations leader who connects daily workflows to the bigger picture — patient access, experience, and community health. You’re comfortable managing across multiple sites and non-standard hours, and you bring strong judgment, clear communication, and a collaborative leadership style to everything you do.

  • Bachelor’s degree in Nursing, Healthcare Administration, Public Health, Business Administration with a healthcare focus, or a closely related field — or equivalent experience
  • Minimum 2 years of supervisory or operational leadership experience in a healthcare setting
  • Demonstrated ability to coordinate staff, workflows, and daily operations across multiple programs or sites
  • Ability to use operational data to monitor performance, identify workflow issues, and support access improvement efforts
  • Strong communication and relationship-building skills across clinical, administrative, school, and community settings
  • Commitment to health equity and culturally responsive care for underserved populations

Preferred:

  • Prior experience in an FQHC or community health center environment
  • Experience supporting urgent care, community health, or school-based health programs
  • Current Rhode Island RN licensure or equivalent clinical credential
  • Bilingual or multilingual skills aligned with the communities served
  • Familiarity with EHR systems (eClinicalWorks, Epic, or NextGen)
Schedule & Locations

This is a full-time (40 hours/week), benefit-eligible position operating across Thundermist sites where Convenient Care, Telephone Triage, and school-based health programs are delivered. Schedule includes regular weekday clinic hours with assigned coverage during evenings, weekends, and holidays as operational point of contact for Convenient Care and extended access programs. Regular travel between Thundermist sites and school-based health sites is required.
